Amyloid-β — a reflection of risk or a preclinical marker?, Published online: 30 April 2018; doi:10.1038/s41582-018-0008-9

In a recent study, individuals with brain amyloid-β accumulation but no cognitive impairment were classified as being at risk of Alzheimer disease, yet amyloid-β is widely considered to be a pathological biomarker of Alzheimer disease rather than a risk factor — it cannot be both.

Neural substrates of social facilitation effects on incentive-based performance

Abstract
Throughout our lives we must perform tasks while being observed by others. Previous studies have shown that the presence of an audience can cause increases in an individual's performance as compared to when they are not being observed—a phenomenon called 'social facilitation'. However, the neural mechanisms underlying this effect, in the context of skilled-task performance for monetary incentives, are not well understood. We used functional magnetic resonance imaging to monitor brain activity while healthy human participants performed a skilled-task during conditions in which they were paid based on their performance and observed and not observed by an audience. We found that during social facilitation, social signals represented in the dorsomedial prefrontal cortex (dmPFC) enhanced reward value computations in ventromedial prefrontal cortex (vmPFC). We also found that functional connectivity between dmPFC and ventral striatum was enhanced when participants exhibited social facilitation effects, indicative of a means by which social signals serve to modulate brain regions involved in regulating behavioral motivation. These findings illustrate how neural processing of social judgments gives rise to the enhanced motivational state that results in social facilitation of incentive-based performance.

Impacts of Mauritia flexuosa degradation on the carbon stocks of freshwater peatlands in the Pastaza-Marañón river basin of the Peruvian Amazon

Abstract

Tropical peat swamp forests (PSF) are characterized by high quantities of carbon (C) stored as organic soil deposits due to waterlogged conditions which slows down decomposition. Globally, Peru has one of the largest expanse of tropical peatlands, located primarily within the Pastaza-Marañón river basin in the Northwestern Peru. Peatland forests in Peru are dominated by a palm species—Mauritia flexuosa, and M. flexuosa-dominated forests cover ~ 80% of total peatland area and store ~ 2.3 Pg C. However, hydrologic alterations, land cover change, and anthropogenic disturbances could lead to PSF's degradation and loss of valuable ecosystem services. Therefore, evaluation of degradation impacts on PSF's structure, biomass, and overall C stocks could provide an estimate of potential C losses into the atmosphere as greenhouse gases (GHG) emissions. This study was carried out in three regions within Pastaza-Marañón river basin to quantify PSF's floristic composition and degradation status and total ecosystem C stocks. There was a tremendous range in C stocks (Mg C ha−1) in various ecosystem pools—vegetation (45.6–122.5), down woody debris (2.1–23.1), litter (2.3–7.8), and soil (top 1 m; 109–594). Mean ecosystem C stocks accounting for the top 1 m soil were 400, 570, and 330 Mg C ha−1 in Itaya, Tigre, and Samiria river basins, respectively. Considering the entire soil depth, mean ecosystem C stocks were 670, 1160, and 330 Mg C ha−1 in Itaya, Tigre, and Samiria river basins, respectively. Floristic composition and calcium to Magnesium (Ca/Mg) ratio of soil profile offered evidence of a site undergoing vegetational succession and transitioning from minerotrophic to ombrotrophic system. Degradation ranged from low to high levels of disturbance with no significant difference between regions. Increased degradation tended to decrease vegetation and forest floor C stocks and was significantly correlated to reduced M. flexuosa biomass C stocks. Long-term studies are needed to understand the linkages between M. flexuosa harvest and palm swamp forest C stocks; however, river dynamics are important natural drivers influencing forest succession and transition in this landscape.

Consistent estimation of polychotomous treatment effects with selection-bias and unobserved heterogeneity using panel data correlated random coefficients model

Abstract

We estimate multiple treatment effects in presence of selection-bias and response heterogeneity, using panel data. A control function was added to a fixed-effects based correlated random coefficients model. Selection model to create the control function was contrasted between multinomial logit and multinomial probit. For the multinomial logit model, parametric and semi-parametric bias correction techniques, as proposed in Lee (Econometrica 51(2):507–512, 1983), Dubin and McFadden (Econometrica 52(2):345–362, 1984) and Dahl (Econometrica 70(6):2367–2420, 2002) respectively, were implemented. We find that controlling time-varying endogeneity, allowing response heterogeneity, the type of bias correction method and the choice of the selection model, each had significant impact on the estimated treatment effects. Using the case of biologic DMARDs, we show that in the presence of heterogeneity and multiple treatments, the specification of the latent index model should be carefully chosen along with selection bias correction techniques appropriate to the choice of the latent index model. These issues have an important impact on policy. Under one set of assumptions, we may accept a formulary expansion policy on biologic DMARDs to be cost-neutral, while rejecting the same policy as not cost-saving under another set of assumptions.

POWIFF- Prospective study of wrist internal fixation of fracture: A protocol for a single centre, superiority, randomised controlled trial to study the efficacy of the VRP (2.0) distal radius plate (Austofix) versus the VA-LCP (Depuy-Synthes) for distal radius fractures

Abstract

Background

Distal radial fractures are one of the most common orthopaedic cases that present to the A&E department. Surgical intervention is warranted in displaced intraarticular fractures and fractures with more than the recommended angulation or shortening, and is most commonly treated with volarly placed fixed angle locking plates. The aim of this study is to determine and compare the efficacy of two different plates for surgical treatment of distal radius fractures. The VRP 2.0 is a new plate produced by the Austofix company and this system will be compared against the VA-LP (Variable angle-locking plate) produced by Depuy-Synthes which has been used as the standard treatment device.

Methods and Design

Patients between the ages of 18 and 80 presenting to the Royal Adelaide Hospital with isolated closed distal radial fractures will be invited to participate in this study. A total of 200 patients are required to provide 90% statistical power at a 5% alpha level to detect a difference of 11.5 points on the PRWE (Patient rated Wrist evaluation) score. The primary outcome measure will be the PRWE score while the secondary outcome measures will include the DASH score, EQ5D score, clinical range of movements, grip strength as well as patient perceived return of function at the wrist and time to resumption to work. These will be measured at 6 weeks, 3 months and 12 months. Radiographic indices including the radial tilt, length, volar inclination and plate prominence will also be measured. Complications will be recorded up to 12 months. Post hoc comparisons will be done using paired t tests. An intention to treat and a per protocol analysis will be done to compare the 2 groups.

Discussion

Distal radial fractures are increasingly being treated by internal fixation using volar locking plates. However, there is no prospective study to date comparing one plate against another in terms of outcome and complications. This study could provide more information about the best way to treat these injuries surgically.

Trial registration

The trial is registered with the Australia New Zealand Clinical Trials Registry (ANZCTR). Trial registration date-17/11/2016. Trial registration number-ACTRN12616001590459.
